Terms and Conditions for Stroke Recovery Coaching Services

1. Introduction

These terms and conditions govern the provision of stroke recovery coaching services (hereinafter referred to as “Services” by Recover With Elena (hereinafter referred to as “the Coach” to the individual or entity engaging the Services (hereinafter referred to as “the Client”.

2. Services Provided

The Coach agrees to provide stroke recovery coaching services to the Client.
These services may include but are not limited to:

  • Developing personalized stroke recovery plans.

  • Providing guidance on where to obtain physical rehabilitation exercises.

  • Offering emotional support and encouragement.

  • Educating on lifestyle adjustments for post-stroke recovery.

  • Monitoring progress and making adjustments to the recovery plan as necessary.

3. Client Responsibilities

The Client agrees to:

  • Provide accurate and complete information regarding their medical history, current condition, and any other relevant information necessary for the provision of Services.

  • Follow the recommendations and instructions provided by the Coach to the best of their ability.

  • Notify the Coach of any changes in their condition or circumstances that may affect the provision of Services.

4. Confidentiality

The Coach agrees to keep all information shared by the Client confidential unless required by law to disclose it. This includes all personal, medical, and other sensitive information sharedduring the coaching relationship.

5. Fees and Payment

The Client agrees to pay the agreed-upon fees for the Services as outlined in the coaching agreement. Payment terms will be specified in the agreement, including any upfront payments, instalment plans, or other arrangements.

6. Cancellation and Rescheduling

The Client agrees to provide at least 12 hours’ notice for any cancellations or rescheduling of coaching sessions. Failure to provide adequate notice may result in forfeiture of the session or a fee as outlined in the coaching agreement.

7. Limitation of Liability

The Coach is not a medical professional and does not provide medical advice or treatment. The Client acknowledges that the Services provided by the Coach are not a substitute for medical care or advice from qualified healthcare professionals. The Coach shall not be liable for any damages or injuries arising from the Client’s failure to seek appropriate medical care or follow medical advice.

8. Termination of Services

Either party may terminate the coaching relationship at any time with written notice to the other party. In the event of termination, any fees owed for Services provided up to the date of termination shall be payable by the Client.

9. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the Commonwealth of Australia. Any dispute arising out of or in connection with this Agreement, including any question regarding its existence, validity, or termination, shall be subject to the exclusive jurisdiction of the courts of New South Wales in Australia where disputes will be resolved.

10. Entire Agreement

These terms and conditions constitute the entire agreement between the Coach and the Client and supersede all prior agreements and understandings, whether written or oral, relating to the subject matter herein.

By engaging the Services of the Coach, the Client acknowledges that they have read, understood, and agreed to these terms and conditions.
